Secure self-referral to partners
A key challenge facing councils and Integrated Case Boards is implementing cost-effective, integrated services that support people to live healthy, active lives.
Theseus: Wellbeing Self-Help is designed to alleviate service pressures by inspiring and empowering individuals to take ownership of their health and wellbeing needs. Self-management and prevention are better than treatment and cure for the individual, communities and for public services.
Theseus: Wellbeing Self-Help provides an engagement website, a library of self-care resources, social prescribing service directory and secure referrals ecosystem for direct citizen referral to trusted partners, such as health walk groups.
Theseus: Wellbeing Self-Help is designed to help reduce pressures on health services and to enable people to live healthier lives for longer.
The central component of Theseus: Wellbeing Self-Help is a modern, attractive and engaging website to inspire and motivate individuals to adopt healthy, active lifestyles.
The website provides a ‘shop window’ of easy to read, accessible health and wellbeing topics such as smoking, alcohol, weight management and active lifestyles. To save your team time and resource, these topics are pre-populated with established best practice information and linkages to official NHS resources. Your organisation can easily tweak, edit and add to the content via an intuitive editor.
The website also provides individuals with access to an integrated ‘best in class’ social prescribing service directory. The directory has icon-led navigation and an innovative ‘conversational sentence’ for easy exploration without confusing, complex filters. A location-aware map interface makes it easy for citizens to identify support nearby as well as further afield.
The website’s look and feel can be customised to your organisation’s brand and preferences.
Your network of partners can maintain their own directory listings via an easy-to-use administration area. Partners can add imagery, pin map locations and categorise their offers using tags. They also receive automated prompts asking them to check their service details are up to date.
Theseus: Wellbeing Self-Help can support citizen self-referral directly to partners via service directory listings.
A Partner Portal is provided for partners to securely receive and manage their referrals and to share outcomes with your organisation.
Theseus: Wellbeing Self-Help is fully responsive for use of all modern devices and is compliant with WCAG 2.2 ‘AA’ accessibility standards.
Service directory partners are provided with bitesize video training materials for quick and easy onboarding.
Understand the impact of self-referrals made with cuts by citizen demographic, provider and health and wellbeing needs with Theseus: Wellbeing Self-Help’s powerful dashboards.
