Social Prescribing in Knutsford

Knutsford Together launched in 2022 and is passionate about supporting residents through compassion and respect, whether they are in crisis or suffering from social isolation. Knutsford Together takes a holistic ‘whole person, whole community’ approach to support and understands individuals often need help from multiple sources.

Central to the organisation’s work is connecting residents to services via effective signposting supported by a network of Local Connectors.

To assist in its important role, Knutsford Together needed a social prescribing software solution to help manage referrals and an engaging website with a directory of services for residents, relatives and neighbours to explore.

A key part of our selection for the project was our ability to deliver a connected, cohesive solution with a bespoke, tailored website, seamlessly integrated with a professional social prescribing system.

Social Prescribing Directory

At Cyber Media we have a dedicated team of design experts which enables us to develop exceptional digital ‘shop-windows’ for initiatives, directories and support networks.

A key aspect of our approach is the application of User-Experience Design in the implementation of directory of services to ensure citizens are never presented with complicated options that can be cumbersome and confusing.

Instead a ‘conversational’ interface takes the citizen through choices available in stages and builds up a simple filter sentence in plain English that’s easy to understand.

The Audience

The social prescribing directory implemented for Knutsford Together facilitates interaction between four primary user types:

Mobile

Residents

Members of the community and their families can use the website to explore and connect with community assets and services, or self-refer themselves for support.

Caseworkers

Members of the Knutsford Together team can login to Theseus: Social Prescribing to receive referrals from residents, record signposting and process onward referrals.

Local Connectors

Local volunteers are trained by Knutsford Together to make residents aware of the support available locally and connect residents with community assets via the directory.

Local Providers

These are service providers and community assets listed in the directory. Principal providers have unique accounts for Theseus: Social Prescribing to accept direct referrals from the Knutsford Together team and to report outcomes.

Fully Integrated Design

The directory is beautifully integrated in a design that is bright and vibrant to complement the Knutsford Together logo and brand.

Our talented team of designers created a range of custom illustrations to help users navigate the directory and the same illustration style is used in a series of ‘scenes’ on website pages to convey the community ethos and scope of the Knutsford Together offer.

For a project of this nature, accessibility is of paramount importance to maximise reach and inclusivity and the website was developed in accord with WCAG 2.1 ‘AA’ standards.

Social Prescribing Referrals Management

At the heart of our solution for Knutsford Together is our Theseus: Social Prescribing system, a modern, flexible and easy to use system with the following key features

Service Directory

Integrated directory of services – the same intuitive directory interface is available to caseworkers in Theseus

Inbound Referrals

Ability to securely receive inbound referrals from a variety of sources, including GPs, or manually create client records for individuals presenting face-to-face

Custom Wokflows

Customised assessment and case management workflows

Onward Referrals

Facility to securely onward refer clients to trusted providers and receive outcomes

For Knutsford Together we configured Theseus: Social Prescribing to support two distinct types of interaction. For some residents, it is most appropriate for the team to simply signpost the resident to a local asset and in these cases the Knutsford Together records a signposting note, with details of what was discussed, against the client record.

For other residents the Knutsford Together team needs to directly refer the individual to a provider organisation. To facilitate this the directory view in Theseus highlights community assets that receive referrals and a member of the team can simply select these to send a referral, accompanied with a note.

Outcome Reporting

Theseus automatically prompts providers to report outcomes for referrals received within custom timeframes.

For Knutsford Together we configured a prompt for community assets to provide an outcome 28 days following receipt of a referral. The outcome can be viewed by the Knutsford Together team within the client record to gain an understanding of whether the referral had a positive impact for the resident.

The Knutsford Together team can also complete assessment forms at different stages of engagement with a resident. A WEMWBS (The Warwick-Edinburgh Mental Wellbeing Scale) form was deployed and the team can complete an assessment with a resident at the start, during and at the end of a period of engagement to understand progress.
