Theseus: Physical Activity is a physical activity case management system that empowers healthy lifestyles teams to assess, manage and refer individuals to providers and facilities in their locality.
Physical Activity assessments
Flexible exercise pathways
Onward referral to exercise providers
Theseus: Physical Activity is for local authority teams offering active lifestyles behaviour change programmes. District, County and Metropolitan sport and exercise networks are supported and physical activity COM-B behavioural change rules are readily available.
Theseus: Physical Activity supports behavioural change triage and features integrated assessment tools including WEMWBS, PAR-Q and IPAQ. It can also support staff with automated (and definable) onward referral suggestions based on a citizen’s age, health, interests and locality.
A Flexible Form Builder enables your service to create dynamic, pathway-specific assessments and follow-up prompts.
If your service works with a network of external exercise providers - such as aerobic classes or walking groups - our Partner Portal Module enables easy onward referral of individuals to your partner providers.
The Partner Portal Module is optimised for use outside of traditional desk-based environments. Exercise providers can use the portal on mobile and tablet devices in community settings to process referrals received from your service.
Exercise providers are prompted by the portal to report outcomes so your service can understand impact and plan service delivery.
To help maximise engagement with physical activity behaviour change initiatives we offer engagement website and client portal options.
Our engagement website offer includes brand development, promotion of self-help resources and a self-referral or self-assessment facility into your service.
The Client Portal Module helps to sustain engagement and citizens can use it to review upcoming appointments, set goals and maintain a journal.
Theseus: Physical Activity’s Flexible Form Builder enable your service to create assessments and follow-up forms tailored to your local requirements.
Empower citizens to self-serve with an active lifestyles engagement website and service directory. The directory can help citizens identify activities and exercise groups in accord with their needs.
Two-way integrations are available facilitating direct referral from EMIS, SystmOne and Vision primary care systems, with write-back of outcomes to the patient record.
