The Outcome Form Builder allows you to create an unlimited number of scored assessment forms, each supporting up to 30 questions. Alongside every scored question, staff can add free-text “evidence” to provide additional context or detail.
Scoring is automatic – the system instantly calculates totals based on responses, removing the need for manual checking and ensuring accuracy.
Common assessments such as SWEMWBS, AUDIT, DUDIT and other validated tools can be replicated in Theseus, while custom assessments such as risk profiling or service-specific outcome measures can be designed to your exact requirements.
Results are presented in easy-to-read radar graphs, giving staff and clients a clear picture of outcomes at a glance. When assessments are repeated, Theseus can overlay multiple radar graphs for comparison, or plot line graphs showing progress over time for individual questions.
To ensure important information is never missed, visual alerts can be configured in the client info panel. These alerts show the score of the latest completed outcome form and can use up to five colour bands to distinguish different score ranges – for example, low, medium, or high risk.
Outcome forms can be embedded directly into custom episode sections, where progress charts are automatically displayed alongside other client data. They are also fully compatible with the client portal, allowing citizens to complete assessments online and track their own progress through time.
